Concord Reserve Assisted Living offers a full range of services for our residents. Suites are rented month-to-month.
Concord Reserve strives to meet each resident’s needs while maintaining the highest level of choice and dignity. This plan is based on your interests and requirements. It is reviewed periodically and can be changed as needed.
Basic Services Offered To All Concord Reserve Residents Include:

- Weekly housekeeping and laundry
- 24-hour on-site nursing staff
- Emergency response system
- Scheduled transportation
- Life enrichment activity programs
- Flat screen television with over 30 basic cable channels included
- Individual heat and cooling control in suite
- A wide variety of spiritual life programs and services
- Outdoor parking is available to our residents who drive
Typical Care Plan Services May Include:
- Medication management
- Mealtime reminders
- Incontinence care
- Escort assistance to and from meals and activities
- Additional housekeeping services
- Health services: monitoring, supervision or assistance
- Select diabetic care
- Assistance with personal care such as bathing, dressing and grooming
- Periodic nighttime assistance
Additional charges apply based on the personal assistance services provided. Physician services are available through our Wellness Center and rehabilitation services are available through the Center for Advanced Rehabilitation.
Our Assisted Living has two specialized neighborhoods which include the Augsburg and the Coburg. The Augsburg has 18 private suites and walk in European showers, private dining areas and beautiful common areas for residents, families, and visitors to enjoy. The Coburg has 25 private suites and is secured. We have special memory support programs created to maintain independence, dignity ans support creativity with art and music therapy.
