Lutheran Home at Concord Reserve is an independent not-for-profit organization that has been dedicated to providing excellence in health care, rehabilitation and senior living services for nearly 80 years. Lutheran Home at Concord Reserve is open to individuals of all faiths and is a 192 bed skilled nursing facility that accepts Medicare, Medicaid, Private Pay and several private insurances.
Our Health Center has been recently renovated throughout the neighborhoods and resident rooms to provide a more comfortable and home-like environment. Most rooms are exclusive, featuring private bathrooms and window views of the landscaped grounds.
The Ohio Department of Health has recognized Lutheran Home at Concord Reserve for our exceptional elder care and professionalism by awarding us a
“Perfect Survey” in the Health Center and Assisted Living. This is presented to 5% of all facilities in the State who achieve a deficiency free survey.
Our neighborhoods are comprised of private and semi-private rooms with up to 38 residents calling them “home”. Each has unique color schemes and décor with common areas for our residents and families to enjoy together. The spa rooms feature heated towel racks that are both relaxing and comforting. The private dining room on each neighborhood provides delicious and healthy meals prepared under the direction of our creative and talented Food Director. Residents are offered multiple food selections that are sure to please every appetite. Each neighborhood has Life Enrichment staff who involve all residents in daily unit activities and campus presentations, including Art, Music and Recreational Therapy.
